By entering a payment plan and membership with us you agree to the following terms and conditions:
The BREATHE SKIN BODY MIND Membership Program is non-transferable, non-refundable, and cannot be cancelled within the agreed upon term.
This membership contract is for a minimum term of 12 months; during which payments will be debited in a frequency of WEEKLY.
Failure to make payments may result in the remainder of your contract value being referred to debt collection, you must notify us as soon as possible regarding a change in circumstances or in the case of financial hardship.
If there are any overdue amounts on your account, you cannot continue your treatments with us until this is paid.
By entering the BREATHE SKIN BODY MIND Membership Program, you acknowledge the risks associated with facial treatments, and therefore you indemnify, hold harmless, and will not sue BREATHE SKIN BODY MIND for any physical or emotional harm, injury, and cosmetic defect arising from these treatments.
As a member you will not be required to place a booking deposit; however, a no-show or cancellation within 24 hours of your appointment may result in forfeiture of the scheduled treatment and this may be deducted from your account. Alternatively, you may be charged a $50 rescheduling fee.
We request that you arrive 15 minutes prior to the start time of your appointment to allow for photos to be taken. The before and after photos are compulsory and required to be taken by your therapist at the beginning and end of your membership.
If you arrive late to your appointment this will be deducted from your treatment time and if you arrive more than 15 minutes late your appointment will be forfeited and deducted from your account.
It is understood that the amount payable on your membership will only cover the agreed upon treatments. If you wish to have extra treatments outside of those prescribed, they must be paid for. All treatments will be pre-booked. All treatments must be used within the term of the membership, if you finish your treatments before the conclusion of your membership term you must continue to make payments on your account.
